This is a basic life policy through The Standard and is

provided at no cost to the employee. The value of this

policy is one times the annual salary rounded up to the

nearest thousand but not exceeding a $125,000.00 value.

This policy can be converted once an employee leaves

The City. The initial application for portability must be

received by The Standard within 31 days.
